Botox vs. Dermal Fillers: What’s the Difference and Which One Is Right for You?
If you’ve ever looked in the mirror and wondered, “Do I need Botox, filler, or both?” - you’re definitely not alone.
One of the most common questions I hear in aesthetic medicine is the difference between Botox and dermal fillers. They’re both injectable treatments, both can create beautifully natural results, and both can be part of a thoughtful anti-aging plan.
But they work in very different ways.
At LAVOUCHE MedSpa in Vienna, VA, our philosophy is simple: your treatment should look like you - just refreshed, rested and beautifully balanced. The goal isn't to change your face. It's to understand how your face is aging and choose the right tool for the right concern.
Botox and fillers don't do the same thing
Let's start with the easiest way to remember it:
Botox relaxes muscles. Fillers restore or enhance volume.
Botox and other neuromodulators work by temporarily reducing the muscle activity responsible for certain expression lines. This is why they are commonly used for areas such as forehead lines, frown lines between the brows and crow's feet.
Dermal fillers are different. They are gel-like injectable materials designed to add volume, smooth certain folds, or enhance areas such as the lips, cheeks, chin, jawline and under-eye area, depending on the specific product and indication.
The FDA regulates dermal fillers as medical devices and notes that most FDA-approved fillers are temporary and are gradually broken down by the body.
So… do you need Botox?
Botox may be something to discuss if your primary concern is movement-related lines.
Think about what happens when you raise your eyebrows, frown, smile or squint. Over time, repeated muscle movement can contribute to lines becoming more noticeable even when your face is relaxed. A properly planned neuromodulator treatment can soften the appearance of those lines while maintaining facial expression.
And this is where I think aesthetics gets interesting.
More isn't necessarily better.
The goal isn't to eliminate every movement in your face. Your expressions are part of what makes your face yours. A good aesthetic plan should consider your anatomy, muscle activity, facial proportions and what you actually want to accomplish.
When might filler make more sense?
Filler is often considered when the concern involves volume loss, contour or structural support rather than simply muscle movement.
For example, depending on the individual patient, filler may be used to address:
Lip volume and definition
Cheek volume
Chin projection
Jawline contour
Under-eye hollows
Facial volume changes associated with aging
Dermal filler treatments should be individualized. Different filler materials have different characteristics, longevity and approved uses. The American Academy of Dermatology also emphasizes that filler injections are medical procedures and that provider experience matters when it comes to both safety and achieving natural-looking results.
What about the “I don't want to look done” concern?
This is probably one of my favorite conversations to have with patients. You don't have to choose between aging naturally and looking completely different.
There is a lot of space in between.
My approach is to look at the whole face rather than automatically treating the line or wrinkle that bothers you most in the mirror. Sometimes the best aesthetic treatment isn't adding more volume to the exact place where you see a wrinkle. Sometimes it's addressing another area of the face that has changed with time.
That is why an individualized consultation is so important.
One of the biggest mistakes I see in aesthetics
Chasing individual wrinkles without understanding the bigger picture.
Facial aging isn't simply about wrinkles. It's a combination of changes involving skin quality, collagen, fat compartments, bone structure, muscle activity and environmental factors. That's why the modern approach to aesthetic medicine is becoming increasingly comprehensive.
Instead of asking:
“Where can we put something?”
I prefer to ask:
“What has changed, and what would restore balance?”
That subtle shift can completely change the treatment conversation.
What about skin quality?
Injectables are only one piece of the anti-aging puzzle. Beautiful skin is also about maintaining the quality of the skin itself.
Depending on your needs, an aesthetic plan may include treatments such as chemical peels, microneedling, PRP, collagen-stimulating treatments, Morpheus8, SKINVIVE and a thoughtful home skincare routine.
The American Academy of Dermatology recommends a comprehensive approach to premature skin aging that includes sun protection and an appropriate skincare regimen.
In other words:
Don't just treat the wrinkle. Take care of the skin underneath it.
The bottom line
Botox and dermal fillers are not interchangeable.
Botox is primarily about muscle movement.
Dermal fillers are primarily about volume, contour and structural support.
And neither treatment is automatically “better.”
The right approach depends on your anatomy, goals, skin, age, previous treatments and the specific changes you want to address. At LAVOUCHE MedSpa in Vienna, VA, we believe the most beautiful aesthetic work is often the work that doesn't announce itself.
Someone may simply say:
“You look amazing. Did you do something different?”
And honestly?
That's the compliment.
Thinking about Botox or filler?
If you're curious about what would actually make sense for your face, start with a consultation rather than starting with a syringe. We'll look at your facial anatomy, discuss your goals, talk through options and create a plan that makes sense for you.
Beauty should never be one-size-fits-all.
